These prices are effective as of May 1, 2011, but are subject to change without notice.
The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any cremation arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy an item you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the goods and services you selected.

In an effort to keep our cremation prices as low as possible, families are asked to make cremation arrangements over the telephone. For those needing to make arrangements in person, we will include a $95 fee to the service you select to offset additional staff charges. |

Cremation Packages
All of our cremation package plans include: basic services of licensed professional and staff, transfer of remains anywhere in our service area, medical examiner fee, use of refrigeration, necessary authorizations, cremation container and crematory fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The container we provide is a sturdy double walled corrugated cardboard container which is already included in our direct cremation charge. If the consumer brings in their own container subtract $15 from the member and non-member prices below.

Direct Cremation Package This is our simplest offering and is designed for those who "just want to be cremated" without any service or ceremony. This basic package includes everything legally required for a direct cremation as mentioned on the previous page. This package does not include an urn.

Veterans Cremation Package This cremation package was specifically developed for veterans in order to take full advantage of all the veterans benefits available at the time of death. It provides a dignified tribute to those that proudly served our country and is offered at a discounted rate as our way of saying thank you on behalf of a grateful nation.

In addition to all of the services provided in our direct cremation package, the veteran package includes:
- Solid bronze veterans urn with branch of service insignia
- Coordinating and directing a committal service in a veterans or other local cemetery
- Scheduling military honors, including flag folding detail and taps, conducted by military personnel (if desired)
- United States flag and weatherproof flag case
- Submission of local obituary notices (if desired)

Cremation Service Options
| Private Family Viewing |

Immediate family members (no more than ten) may view the deceased for up to one hour in one of our viewing rooms. We are regrettably unable to offer private viewing when an autopsy has been performed.

| Family-Witnessed Cremation |

For those who want the peace of mind of being present for the cremation process, we will schedule the cremation at a mutually agreed upon time during normal business hours. It is our policy that an upgraded cremation container is required.

Cremation Merchandise Options
| Nationwide Coverage and Protection Plan |

For our members who travel, or temporarily or permanently move out of our service area, this unique extended service plan guarantees our continuity of care regardless of where our services are needed in the United States. If death occurs anywhere in the county, your family simply calls us and we will make all the necessary arrangements for the cremation to be performed at our member rate and at no additional charge to your family. Our Nationwide Coverage and Protection Plan can only be selected at the time of membership and your family must call and notify us as the time of death so that we can contact the cremation provider of choice in our network.

For Private Viewing or Family-witnessed Cremation, an upgraded cremation container is required. Our upgraded container is composed of grey fiberboard with a one piece cover and lined interior.

Unless the family wishes to purchase a permanent urn, we will return the cremated remains in a temporary cardboard container. We display over 100 urns in our urn selection room. Color pictures of these can be found on the website.

| Electronic Obituary Placement |

As local newspapers continue to charge significant fees for obituaries, people are relying more and more on on-line obituaries. This fee is for placement of the obituary on our csnh.com website for twelve months, providing family members and friends access to a full, unedited obituary, on-line guestbook, photo gallery and email notification seen by over 3,500 on-line visitors each month. The obituary is also placed on Tributes.com, the country's largest obituary database.

Disposition Options
| Release of Cremated Remains |
We will release the cremated remains to the family at one of our facilities during regular business hours. After 30 days, there will be a fee of $85 for sheltering and storage of cremated remains. This fee will be assessed per year or portion thereof.

We will carefully package and ship the cremated remains via USPS Registered Mail to any domestic destination. (Please note that shipment may take up to 5 business days from date of cremation.) For safety and security, it is our policy that an urn is required for shipment of cremated remains.

| Coastal Scattering of Cremated Remains |

| We will coordinate an unattended scattering off the coast of NH near
the Isles of Shoals conducted by New England Burials at Sea. Fee includes
photo documentation and certificate of the scattering mailed to the family
as well as EPA filing. Other locations with and without family in attendance
are available.

Membership Requirements
|
To be a member and eligible for member discounts and benefits, the completed membership form and registration fee must be received by our office at least 48 hours prior to a member's passing.
|
Credit & Payment Policy
For those who have not prepaid for their services, we have established a uniform credit policy to serve all families fairly and to alleviate misunderstandings. A sound credit policy enables us to contain our costs to all the families we serve. Our general credit policy is that the entire amount of the cremation is due and payable at the time services are rendered. We accept cash, personal and bank checks, and credit cards (Visa, MasterCard, Discover and American Express) as forms of payment. We regret that we are unable to accept settlement of estates through attorneys or probate court as a means of financial arrangements, nor are we able to process life insurance policies. All unpaid balances after sixty days (60) from the date of service accrue a finance charge of 1% per month (12% per year)
The General Price List does not include prices for certain items you may ask us to buy for you such as cemetery charges, newspaper notices, clergy/church fees and fees of other funeral homes. The prices for those items will be shown on the statement describing the cremation goods and services you selected. |
